Usually, you will need to upload a copy of the receipt, a picture of the UPC, and the rebate form. The rebates I have received have been by way of a mailed prepaid card. Don't forget to use Ebates and your points credit card when making your purchases!

You can also do a google search and type the store name and the word "rebates".
